Helpful Forms

If you're a new client, please complete the following forms and bring them to your first therapy session.

• New Patient Information Sheet
• Outpatient Service Agreement
• Biopsychosocial History Form (if Adult)
• Child Developmental History Form (For patients under 18)

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information:

Also, if you would like to utilize a credit card for payment, please complete the Credit Card Payment Consent Form, which is required to be on file.
